The Sandusky County Common Pleas Court, Domestic Relations Division, recognizes that legal matters can be complicated and may be best handled by an attorney.

These forms are available for your convenience and can be utilized if you are not represented by an attorney and representing yourself.

The forms below can be completed online and printed or printed and completed by hand.  You are responsible for reading and understanding all instructions for these forms.  Forms can also be obtained at the Sandusky County Clerk of Courts office, located at 100 N. Park Avenue, Suite 208, Fremont, Ohio 43420.  

Any forms you wish to file with the Court must be filed with the Sandusky County Clerk of Court’s office located at 100 N. Park Avenue, Suite 208, Fremont, Ohio 43420.
